How Can I Add A Contact us form/page to my website?

What would be the best way to add a contact form/page to my website? I wanna add Another Tab at the end "Contact Us" with a basic PHP contact form.

Just create a custom page. If the contact form is HTML you could just paste it into the content box. If it's PHP you can change the box type to 'HTML/PHP' and link to an external file with the form. You can read more about custom pages in the AOM Blog.

You actually can get the code from your host provider. Then, create a simple html or php contact form, add a custom page, and link to your store. A custom page usually has a permanent link such as http://www.yourstore.com/page-contact.html.
